Author Description

This text builds upon the findings of a companion study of care management and health care of older people. The chapters consider key elements relevant to the relationship between secondary health service provision and intensive care management for older people. These include the relationship between health and social care, assessment and rehabilitation in the community, standardized assessments, and the role of cost information decision-making.
