Alice finds herself in a bizarre, dreamlike world where she must choose between several potential romantic partners, each representing a different aspect of the 'Joker' persona. Alice's objective is to navigate this strange land and make a choice that will determine her fate, all while dealing with the eccentric and often dangerous inhabitants. The world operates on dream logic, and the characters' motivations are frequently tied to their symbolic roles. The narrative framework appears to be that of a reverse harem, focusing on Alice's interactions and developing relationships with multiple male leads.

The 'Alice in the Country of Joker' manga series is known for its unique take on the classic Alice in Wonderland narrative, blending fantasy elements with a strong romantic focus. Readers often praise the intricate character designs and the compelling, albeit unconventional, romantic entanglements Alice finds herself in. The series is noted for its surreal atmosphere and the psychological depth explored through its characters' interactions. While the premise might seem unusual, it offers a distinct experience within the reverse harem and fantasy genres.
