8 1/2" x 11" Paperback notebook with 80 pages (365 days/1 Year) to keep track of chemotherapy side effects and daily symptoms. Each page has 5 UNDATED days where you can notate the date and EACH DAY HAS A LONG LIST OF SYMPTOMS THAT YOU CAN CIRCLE. This simple method makes it easy to identify symptoms fast with a circle without having to write them down. At the bottom of each page is extra space to keep track of additional symptoms, liquid intake and medicine intake that helped the symptoms (or didn't help) and for additional notes. At the back of the book is 7 blank pages which can come in handy to keep track of doctors names/phone numbers, insurance info.
